Applications for assisted living facilities

Before applying for placement at an assisted living facility, the applicant must undergo a needs assessment for placement. The applicant's medical and social condition is examined, and an assessment made of his or her need.

Needs assessment for assisted living

  • Applications for placement are not made directly to assisted living facilities. Instead, an assessment must be completed to determine the need for placement.
  • Needs-assessment applications for assisted living are submitted on a special form which are available from from the municipal offices for social services.
  • The application is submitted to the municipal social services. The application is then reviewed to decide whether the applicant can continue in his or her own housing with increased support. At this point the process comes to an end.
  • If it becomes apparent that increased support will not meet the applicant's needs, the application is forwarded to a special assessment team, which performs the assessment.
  • The assessment team has six weeks to perform the assessment. At the end of this period the applicant is presented with the conclusions. If the applicant is deemed not to be in need of assisted living, the decision must be qualified.
  • If the applicant is assessed as having demonstrated a need for assisted living, this conclusion is forwarded to the municipal social services, who present the applicant with the various options available and listen to his or her requests for assisted living facilities.
  • Applicants' needs assessments for assisted living are valid for nine months. If he or she has not been assigned a placement by the end of this period, a new assessment must be performed.

Needs assessment application for assisted living

  • The municipal social services hear applicants' requests for assisted living facilities, once their need for assisted living has been established. Social services then sends those requests to the assessment team.
  • Applications may me made for more than one assisted living centre anywhere in the country, and these are prioritised with reference to the applicants' requests.
  • When a place becomes available, the assessment supervisor provides the facility's administration with names of three individuals, one of whom is offered the place.
  • The municipal social services offer counselling for different options in care and alternative living arrangements.
